School Application Form
Please complete the following application form if you wish for your child to attend St. Philip's.  The application form is the same for both school and nursery. 
Please bring your child's full birth certificate and Baptism certificate, if applicable, when submitting your application.  

Failure to bring these documents could result in your child not being offered a place due to a lack of necessary documents.

If there are no places available you can request that your child be placed on the waiting list.  Your child will be on the waiting list until the end of the school year.  If you wish your child to be on the waiting list after the end of the year, you must contact the school to request your child is placed on it again.

If your child will be joining the Reception class at St. Philip's, you will also be required to fill out a Supplementary Application Form (SIF) alongside the school application form above.  Follow the links below to download the SIF form for the relevant academic year.

The current deadline for appeals has passed for this academic year.  Details on the appeals for the upcoming academic year will be published upon release.
All children can access 570 hours (15 horus x 38 weeks term time only) funded early education from the term following their third birthday.  This entitlement can be extended to 1140 hours (30 hours x 38 weeks term time only) funded early education for elligible families.  For more information on the criteria for elligibility please click here.

High School Information

All applications for places in schools are online, there are no more paper based application forms available.  Parents/Carers should receive information directly from the Local Authority on how to complete the form.  When the form is completed you will receive a confirmation email advising that the process is complete.  If you do not receive this you need to contact admisisions on 0113 2224414 to advise them and check with them or log in again to see if it will let you finish the process.

We are a feeder primary school to the local high schools. Please see below details of the high schools which our children historically attend Year 7 from St Philip's.
